I have the FAX release from 1994 with the cover and standard gold-on-black printing on the CD. The CD insert lists one track as is listed here on the page; however, the CD itself has 6 tracks - 5 of 10 minutes apiece, the 6th at 11:10. This is the same as the re-release of Cymatic Scan on Laswell's label a year later. Does anyone have a copy of the FAX CD that can confirm the number of tracks on the CD?

This is normal for FAX releases. Longer tracks (anything over 40 minutes or so) are usually broken down into chunks of 5 or 10 minutes. This was merely a convenience for the age of CD players, and the overall "track" should still be considered one piece of music. My rule of thumb is, if the track lengths are all exactly the same, the entire recording should be considered one piece of music, and listed as one "track" (and should furthermore be combined into a single track when encoding from CD). If the track lengths differ, the tracks should be listed separately. The way tracks are listed on FAX releases is (as far as I know) always consistent with that rule.
